Required documents: - medical prescription (valid two weeks only from the mentioned date)
                                      - coverage card
                                      - ID or passport
Since you are a  NSSF insured, your full name and full date of birth should be mentioned by your physician on the prescription.
Please do not write anything yourself otherwise it won't be approved by your insurance!
